Quick Guide: Using enzyme alginogels in the management of moisture-associated skin damage (MASD)

5 November 2025

This quick guide outlines the prevention and management of moisture-associated skin damage (MASD) and describes how enzyme alginogels, particularly Flaminal®, can support care. MASD is an irritant contact dermatitis caused by prolonged exposure to moisture sources such as urine, faeces, perspiration, saliva, mucus, and wound exudate. It encompasses four main subtypes: incontinence-associated dermatitis (IAD), peristomal complications, intertriginous dermatitis, and periwound MASD. Although these conditions vary in cause and presentation, best practice management follows five core principles: gentle cleansing with pH-balanced products; controlling moisture using absorbent materials and barrier products; treating fungal involvement where present; protecting the skin barrier; and managing associated symptoms such as irritation and inflammation.

The guide presents a staged pathway for prevention, protection and repair of MASD, ranging from healthy skin through to mild, moderate, and severe damage. At more advanced stages—where the skin is broken, exudate is present, or infection risk is high—Flaminal® is recommended. Flaminal® is an enzyme alginogel composed of a hydrated alginate matrix combined with the GLG antimicrobial enzyme system (glucose oxidase, lactoperoxidase and guaiacol). It provides antimicrobial protection, absorbs exudate, supports continuous gentle debridement, and is safe for both skin and wound tissue. Flaminal® Hydro and Flaminal® Forte can be selected based on exudate level. The gel is applied in a 4–5 mm layer and covered with an appropriate secondary dressing. Because its antimicrobial action occurs within the gel matrix, Flaminal® is non-cytotoxic and has shown no antimicrobial resistance to date, making it suitable for managing moderate to severe MASD.
