Worldwide, the prevention of harm to patients is given much attention since the World Health Organization launched its patient safety programme in 2004 (World Health Organization, 2017), partly based on a report from the UK estimating that 10% of patients in the NHS hospitals experience adverse events (Department of Health, 2000).
